DISABILITY CARE

Who We Help

Communicare has extensive experience in caring for children and adults with a wide range of intellectual, physical and sensory disabilities and additional needs such as ASD, Down Syndrome, Motor Neuron Disease, Huntington’s, Parkinson’s, cerebral palsy, acquired brain injuries and spinal cord injuries.

Communicare is a HSE-approved National Provider of Disabilities Care Support Services.

How We Help

Our experienced and professional Disability Care teams help maintain and promote the dignity, health and independence of our service users through a holistic approach which takes account of their social and psychological as well as medical needs. We place great emphasis on reablement.

Each service user has an individualised, integrated care plan, with multidisciplinary input, which is reviewed and revised on a regular basis.

Communicare staff assist with activities of daily living, personal hygiene, feeding and meal preparation, light house-keeping duties, transport to and from healthcare facilities or day services, companionship, socialisation and prompting the administration of medication.

We also collaborate with our service users’ Circles of Support to ensure that they receive best in class care. Our partners in this include the HSE, RehabCare, NLN, IWA and local Community Healthcare Services.

Where We Help

We care for clients in their own homes where possible, often after working closely with the team at the NRH or other hospitals to plan for and facilitate a safe discharge. Much of our disability care in the community takes place over a medium to long-term basis. In some cases, we provide round-the-clock care to profoundly disabled clients at home for a period of years.

Communicare also operates a number of HIQA-regulated facilities across the country. Some of these focus on short-term respite services, offering service users and their families a welcome break from their usual routines. For those service users who require a longer-term solution, we also provide residential care services in purpose-built or adapted facilities. In either context, our aim is to create modern facilities with state of the art equipment which look and feel like a home from home.
